Patient Rights & Responsibilities
You have the right to...
- Choose your wound care supplier for the supplies you receive
- Be provided with the information about your rights in receiving supplies
- Receive your supplies in a timely manner
- Receive supply information from knowledgeable and trained staff
- Be treated with courtesy and respect
- Be provided with service without discrimination against your handicap, race, color, creed, religion, sex, national origin, sexual orientation, or age
- Be free from physical and mental abuse and/or neglect
- Be given privacy and confidentiality handling information regarding Direct Medical’s HIPAA policies
- Be given your insurance benefits prior to shipping supplies to you
- Voice grievance without being threatened or discriminated against
- Be provided with your patient information if requested in writing by you
You are responsible for...
- Notifying Direct Medical of a change of address, phone number or insurance information
- Notifying Direct Medical immediately if you have a reaction to a product
- Notifying Direct Medical if you are under the care of a home health, nurse or physical therapist
- Notifying Direct Medical immediately if you receive a product in error
- Paying any invoice due to Direct Medical as a result of a product supplied to you by us in coordination with your insurance benefits
- Paying Direct Medical any funds paid to you by your insurance company as a result of a product supplied to you by us
- Following up with your physician on a routine basis to ensure proper documentation is available to prove medical necessity for products received
